A match with the practice's insurer list is not an eligibility check, approval of treatment or guarantee of payment. If you proceed to an appointment request, the office still needs to confirm the appointment.

Before committing to treatment, ask which network applies to your policy and whether the proposed care is included. Plans from the same insurer can have different terms. If you have more than one plan, tell the office so the relevant arrangements can be discussed.

It also helps to understand the part you may need to pay:

• A deductible is an amount you may pay before certain benefits start.
• A copayment is a fixed charge; coinsurance is a percentage share.
• An annual benefit maximum limits what a plan pays during its benefit year.
• Exclusions and frequency limits may affect particular services.
