Modern Touch Dental, P.C. operates this website for our dental practice at 426 Atlantic Ave, East Rockaway, NY 11518. This notice explains how information is handled through this website and the Logika contact, chat, appointment-request, Second Opinion and voice features available from it. It concerns these online interactions; it does not replace the practice's Notice of Privacy Practices for patient health information.
Information you choose to provide
The website's contact form asks for your first and last name, email address, telephone number, an optional general message, and permission for the practice to call or email you about your request. It sends these details through Logika only when you select Send message and the required fields and permission are valid; it does not autosave what you type before submission. An empty form session is created when you send; an interrupted submission may leave an incomplete session. Sending a message does not confirm an appointment or enroll you in marketing or an SMS program. You can also contact the office using the telephone and email links.
The appointment-request flow can collect your contact details, preferred day or time, preferred contact method, and any service, provider, location, insurer name or general message you choose to enter. The practice uses this information to consider your request and contact you. A submitted request is not a confirmed appointment.
The text chat asks for your name and telephone number before starting a conversation. These details and the messages you send are received through Logika so the practice team can respond. Text chat is separate from the automated voice assistant. Closing a chat window does not withdraw messages already sent or delete the practice's conversation record.
The designated Second Opinion flow is different from the general contact form. It asks you to select a treatment topic and provide a reachable telephone number or email address; you may also provide your name and a question. Your inquiry is sent first. You can then choose to upload supporting documents or images, which may contain sensitive health information, for the practice to consider with your request. Uploaded files and their filenames are associated with your inquiry in private Logika application storage for access by authorized practice personnel and providers operating that service; they are not published on the website. Uploading files is optional: leaving that step or an unsuccessful upload does not withdraw the inquiry already sent. Submit only material relevant to the request that you are entitled to share.
Please keep general contact, chat and voice messages focused on arranging contact or a visit. Do not enter medical records, detailed health histories, Social Security numbers, insurance member IDs or payment-card details in those general website features. The designated Second Opinion flow described above is the separate route for intentionally providing a treatment question and relevant supporting documents. Call the office if you are unsure how to provide sensitive information. None of these features is for emergencies; call 911 for a medical emergency.
Voice assistant
Starting the voice assistant requests microphone access through your browser. While the voice session is active, your speech is transmitted through Logika's voice service and processed by its voice-model provider to generate responses. The assistant also receives website context, such as the current page and available sections or actions, to help answer questions and navigate the site. This is automated assistance, not a private conversation with a dentist, diagnosis or clinical advice.
You can stop the voice session or revoke microphone permission in your browser. Stopping the session ends further microphone transmission; it does not undo processing that has already occurred. You can use the website's contact links instead of voice.
Technical information and browser storage
Our hosting, video and application services receive technical information needed to deliver their features, such as IP addresses, request times, browser information and page requests. Logika records operational events used to deliver requests, identify errors and limit automated abuse. Chat uses browser session storage to keep the current conversation accessible within that tab; closing the tab can end your access to it without deleting the practice's stored messages.
This version of the website does not have Google Analytics, Google Tag Manager or advertising pixels installed. That does not mean it produces no technical logs or application activity records. External services opened through map, social-media or other outgoing links operate under their own privacy notices.
How information is used and shared
The practice uses inquiry and conversation information to respond, arrange requested contact, manage appointment requests, consider Second Opinion questions and supporting materials, and maintain relevant communication records. Logika and the providers supporting website hosting, video delivery, application storage and voice processing handle information needed for those services. Authorized practice personnel can access relevant requests, messages and submitted documents. Information may also need to be disclosed where applicable law requires it. This notice does not give permission to use a callback request as marketing consent.
Retention and your choices
Retention depends on the type of record, the purpose of the interaction and applicable recordkeeping requirements; closing a page does not automatically erase a request. Contact the practice to ask about information associated with your website inquiry, request a correction or deletion, or change your contact preference. We may need to verify your identity, and legal or patient-record obligations may limit what can be deleted. This notice does not limit rights you have under applicable New York or federal law.
Parents or guardians should contact the office on behalf of children rather than asking children to use general website forms or voice features. No internet service can guarantee complete security; do not send sensitive health information by ordinary email.
Text-message preferences
The website does not currently provide an active SMS enrollment or withdrawal form. Reading the Opt-in, Opt-out or SMS Terms and Conditions pages does not change a messaging preference. Giving contact details for an inquiry is not SMS consent or permission to share a mobile number or messaging consent with another party for promotional use. Any future text-message feature must explain its actual sender, purpose, service providers and consent choices before enrollment is enabled.
